DesireFacilitatory: Most reliable response — desire improves in hypogonadal patients and in women.

Mechanism

Androgen-receptor action in the hypothalamus and limbic system drives desire and upregulates nitric-oxide synthase centrally and peripherally. Desire responds most reliably; erection least (erection is weakly tied to serum testosterone, strongly to vascular health).

Pharmacokinetics
Onset ~3–6 weeks; full effect ~3 months; reversible.
Reversibility
Reversible on discontinuation.

Sex differences & notes

In women (off-label), the definitive meta-analysis of 8,480 women found testosterone raised satisfying events, desire, arousal, orgasm, pleasure and self-image. No FDA-approved female product in the US.
