Poppers (alkyl nitrites)

⚠ Critical

Absolute contraindication with PDE5 inhibitors (sildenafil, tadalafil): both converge on the nitric-oxide–cGMP pathway and can cause catastrophic, potentially fatal hypotension.

Mechanism

Inhaled nitrites donate nitric oxide, activating guanylate cyclase for rapid cGMP-mediated smooth-muscle relaxation and vasodilation. Relax the anal sphincter and vagina to ease penetration and are reported to intensify orgasm.

Pharmacokinetics
Onset seconds; duration 1–2 minutes.
Reversibility
Acute — but the combination with PDE5 inhibitors can be fatal.

Sex differences & notes

Predominant in men who have sex with men; risks include methemoglobinemia and maculopathy/vision loss.
