Mechanism

Nicotinic-receptor–mediated adrenergic stimulation causes vasoconstriction, endothelial dysfunction and reduced nitric-oxide bioavailability. Impairs erection acutely and chronically, lowers testosterone and harms sperm. Vaping delivers the same vascular harm.

Pharmacokinetics
Acute vasoconstriction within minutes; cumulative vascular damage over years.
Reversibility
Partial improvement with cessation, inversely related to age and baseline severity.

Sex differences & notes

Even a single dose can reduce erectile response; heavy smoking roughly doubles the odds of severe erectile dysfunction in young men.
